This data provides hypertension prevalence data for each Zip Code in Allegheny County. The information was produced by Pennsylvania Health Care Cost Containment Council (PHC4) for 2013-2015.

Things to Know
- The data provided reflects information provided by each health plan that corresponded to the definition used for The Healthcare Effectiveness Data and Information Set (HEDIS) reporting, i.e. people between 18 and 75 years of age who met a standard definition for hypertension and continuous enrollment criteria.
- The Denominator in this data set is the total population in each zip code for all reporting health plans. *HEDIS is a tool used by more than 90 percent of America's health plans to measure performance on important dimensions of care and service.
- Users should be cautious of using administrative claims data as a measure of disease prevalence and interpreting trends over time, as data provided were collected for purposes other than surveillance. Limitations of these data include but are not limited to: misclassification, duplicate individuals, exclusion of individuals who did not seek care in past two years and those who are: uninsured, enrolled in plans not represented in the dataset, or were not enrolled in one of the represented plans for at least 90 days.
